I need help setting up a jotform that will go into an autoresponder

can someone please walk me through creating a new jotform for this?

the person that set the last jotform up isn't here anymore

Do you want to put the link of a new form into the Autoresponder of another one? If that is correct, please do the following:

1) Copy the URL of the new form: https://www.jotform.com/help/401-Where-to-Find-My-Form-URL

2) Then, go into the edit mode of the Autoresponder, and paste the link, example:
