Please note that if you use your for URL or use custom URL of form, it will always include JotForm domain name. 

You can use your own domain name to access form by taking advantage of "Custom Domain" feature but it is only available in enterprise plan. Please click on the following URL to check features of enterprise plan: https://www.jotform.com/enterprise/
