password for protected page: pediatric
Additionally, the submit button gets stuck on "please wait" - see screenshot. I've tried this on multiple devices and browsers with the same result.
Lastly, you'll also see on the screen shot, the text next to the check box is not showing despite it showing on the form builder.

I have tried to access the link that you have provided but it seems that it is password protected. I can see that your form is embedded on your website, and I checked the standalone form and it's working fine on my end: https://www.jotform.com/form/200702881961051
May I ask if you have tried to embed the form using the iFrame code? If not, please try to re-embed the form using the iFrame codes, it could be that the form codes are having conflicts with other codes on your website.

Then let us know if the issue still persists.

I had put in the description that the password is: pediatric
It seems to work on a mobile device but not a computer.

Thanks for sharing the information.
I've been testing your page and the videos on your form seem to be displaying without issues, here are some screenshots about my test:
I'm using Chrome on Windows 10, may you please let us know the browser where you replicated the issue?
Also, kindly try using incognito mode, this loads a tab without adds-on so you can see if there is anything installed that causes the issue as you mentioned it worked on mobile.
