how to create a signup sheet in google forms

I'm trying to use google forms as a signup sheet but couldn't figure out how to do yet. As we move to online education, we'd like to get in contact with each parent to manage this transition. I want to have a signup sheet where parents can select the dates and teachers they are going to be speaking to. We'll be getting signatures here. Another sheet will be used by students and they'll select a term paper subject. It'll be hard to do exams in these conditions. 

I'd like to hear how jotform can help in case google forms cant be used in these cases.

For the signup sheet, you can create a form or use one of the templates. What you basically need to do is to create a form with related fields (i.e. parent name, teacher name, student name, etc.). Depending on the information you need for this meeting, you can add different fields.
