Questions about creating a poll form.

We want to have a poll, where (i) only specific users can vote, (ii) only once, (iii) without been able to forward the vote to another PC/IP address, and (iv) without admin been able to track responses to users but only get the results of the poll. How can this happen?

This is possible with unique submissions and unique question.

If you want your form to be viewable on specific users, you may want to enable the form's password protection https://www.jotform.com/help/579-How-to-Password-Protect-a-Form. You can assure that only specific users who has the form password will be able to access it.
