Can the form be partially filled out now, then more later? (if for example the trouble shooting lasted a number of days) I'm thinking along the lines of commissioning of equipment.

I would suggest you to build an approval workflow. You will need to use notifications & autoresponders, conditions, hidden fields, edit links and the Get Form Page URL widget.
