Hi - for some reason, my form product fields are all horizontal. I haven't been in, made any changes, or anything. HELP!

correctly. It's a much needed feature, though!
So, maybe it's just that template I'm using.

You may try out with a new Form, the Categories option. The existing form of yours is not responsive due to the template:
I could not replicate any issue with the demo form:

This problem should be resolved. I couldn't reproduce the problem due to Custom CSS. But the cloned form worked great with Product Layout. Could you please remove the custom css.
