I have modified a form from a cloned form, even changed the title, and I'm having the PDF attachment come through as the original cloned form. I have since deleted the original form but am still having the form not attach the proper worded PDF.
This is happening to two of my forms (that I know of)

If you have customized your PDF Submission Report, whenever you change something on your form, it will no longer automatically update the PDF Report.
If you want to get the latest changes, kindly delete your current PDF Report and create a new one which by default will have everything that is currently on your online form. Then you can customize it further afterwards.

If you're referring to the PDF Document, here's how:
1. Open your JotForm account and go to the My Forms page.
2. Select the form and click the Menu dropdown.
3. Select the Create PDF Document.4. To delete the PDF document, please follow the screencast guide below:
