Thank you for contacting our support team.
In order to reproduce the same table style question in JotForm, you will need to use the Input Table field.
You will be able to receive all answers in plain text, or you can enable the Multi-Type Columns that allow for various other field types within the same table.
Here is a very helpful user guide that will assist you in setting up the Input Table field with multi-type fields in your form: Input-Table-s-New-Type-Multi-Type-Columns

Also, the last row needs to be mutually exclusive. Is that possible?

Unfortunately, there is no option for that in the Input Table. The first column can only be a header and users cannot add texts to it.
A workaround is to use the Spreadsheet widget instead. Here's a guide on How-to-Set-Up-the-Spreadsheet-Widget. But this will prevent you from showing/hiding fields based on the content added to the widget as you cannot add them to the conditions.
