Create Fillable PDF Form

Hello,

I need to know how to make a form that a person can fill out and send back to me filled out. I need to be able to skip the printing step. What I am trying to do is get  patients to fill out a medical update form and for us to get the answers, without them printing the form. Any suggestions on how to do this? I have added a link to the form that has to be printed, but many people are trying to use their phones and cannot print form their phones.

I have not found the instructions on how to use jotforms, either. Can you direct me to that?

If I understand correctly, you would like to create a fillable form from the PDF you have linked.

If so, you can go to this link.

Here, click on the blue "Convert PDF to fillable form" button and upload your pdf:

You can reach this also from your My Forms page.

After that, I would suggest you follow this guide on how to connect the fields of your PDF and the online version of the form.

When you send the link to someone and they fill out your form, you will get the responses in email, and you can also view them later on your form's submission page.
