You need to simply click Use Template button and it will be added to your account. Then you can make any desired customizations and use for your needs.

Your form will have a unique URL (link) that you can copy and share to anyone (emailed and/or texted to your users). Please check this guide below:

However, please note that under 'free' plan account, you will be able to receive maximum 100 submissions per month from the total of 500 submissions. Please check limits and features for our plans:
