Hi there
Im am absolute novice when it comes to computers and tech stuff so please excuse the potentially dumb question im about to ask.
If i upload my medical form to Jot, does this mean my form will be accessible to fill out online and emailed back? or is it still a print and hand to me and or scan back to me?

Hello elementsoffitness,
If you embed your form a web page or send it by direct email, your form will be filled by the form fillers and you will be able to see their answers in your email inbox or JotForm inbox. So, they don't need to print it.
To give an example, I create an appointment form and embed it on my web page. I will be able to see the answers as shown below in my e-mail inbox.

To import your form to JotForm, please follow the steps below:
1) Click on Create Form.
2) Click on the Import Form.
3) Navigate to Import PDF Form.
