Trying to limit entries to once per day

I found this thread and am trying to implement this solution but it is not working for me.

I even cloned her form and set up the fields exactly as in her example. But the unique question field does not get filled, despite making the conditional logic exactly as in her example (see screenshot of mine). 

Anyone know what I'm missing? Or has something changed since March that makes this not work anymore?

I checked your form and condition. You have not made the fields in the condition "brown". So, it does not pass the values of the fields correctly.

Please try to click on the fields in the condition and make them "brown" and then test again.
