Am I suspended forever?

My account was suspended due to suspected phishing. I had an encrypted credit card authorization form for clients to fill out  to pay for their orders. If my account can never be reinstated, could I at least be emailed the contents of my main form because I don't have that text saved anywhere and really need it please. 

Ok I looked through the forums and I just need to delete the form that collects the credit card info. That is fine, could someone please delete it so I can have my account reinstated please thank you
