Hello,

I can see that your Corona responder application is approved and you have unlimited account usage ability until Aug 1, 2020.

But, since you are on the Free account, JotForm branding will show up on your forms. The only way to remove this branding is to upgrade to any paid plan - How-to-Upgrade-My-Account.

Do you want to use your domain name on the form URLs? If yes, then that feature is only available under our JotForm Enterprise plans - https://www.jotform.com/enterprise/

Hi I am trying to remove the Jotform brandname and use our url as well

If you upgrade to any of our paid plans, you can remove JotForm branding by going to your My Account > Settings page, then uncheck JotForm Branding.

However, if you want white labeling as well as use custom domains, that is only available if you upgrade to our Enterprise Plan.

Yes, it will be available on your account even if you upgrade.

The unlimited access will be available till Aug 1, 2020 (even if you upgrade your account or remain on Free).

Enterprise plan is $79/user per month with a minimum of 5 users and it is billed annually. For nonprofits, there will be a 30% discount.

More or less it will be at $3,318 per year. Please note that there is no option for monthly payments.
