How can I create a form that has conditional output to a PDF?

    I am trying to create a form that will allow me to ask a question "Would you like more information about any of the topics below" and have a checkbox next to each option. If a box is checked the corresponding output would be a link.

     

    Form                                           Output

    Option 1 box checked                URL 1

    Option 2 box no checked            Blank

    Option 3 box checked                 URL 3

     

    The output could be either an email that gets sent or maybe a PDF that gets generated and emailed.

    Generally, the emails or the PDF documents include the input fields on the form. However, based on your requirement, it seems that you want to display the output only for the selected options. Please see the demo form below setup with assigning calculation values and a condition to see if that works.
