Why was my account suspended?

    Can we get this account back up. Its for a business. I can provide more information on why we need the information. We are trying to utilize your program to create forms for our clients.

    Unfortunately, I was unable to find an account related neither to the email address you've used to post this ticket or the email address you provided. 

    May you please provide us the JotForm account username or the link to a form in your account, so we can check further on our end?

    Hi, the account "CDFI" was suspended since the form was asking for sensitive information such as social security numbers, and drivers license, which violates our terms of use: https://www.jotform.com/terms/

    "Phishing. You agree that JotForm Inc. may terminate your JotForm Service immediately if a form’s purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately."

    Your account is active now. We had to delete those fields in order to reactivate the account.

    If you need to add those fields again, you can do it if you comply with the following security practices:

    1) Encrypting the form, for which you need to make sure to read this guide before using this feature: https://www.jotform.com/help/344-Encrypted-Forms-and-How-to-Use-Them

    2) Adding a signature fieldhttps://www.jotform.com/help/433-How-to-Add-an-E-Signature-to-Your-Form

    3) Add terms, so the user agrees to provide that sensitive information. You can use the Terms and Conditions, or the Short Scrollable Terms widget for this purpose.
