Form not working

Hi, I have a form that has been embedded in my website for some time now and it is all of a sudden not working.  I attempted to fix it by paying for an upgrade (the storage was maxed) and that has not seemed to help.  I am trying to delete it and re-embed it into my website but I am not sure how I did it in the first place... I am assuming there is some link I have to copy into it but I can't figure out where to find that link.

Hello Kimberly - I visited your website and your embedded form seems to be working fine.

Could you please tell us how your form isn't working? Did you see any error message? If so, kindly send us screenshots to better illustrate the issue.
