Form field missing from record

    on my form COVID-19 Testing and Consent, the form field called Date of Testing does not show up in submissions or in PDF.

     

    It was showing up before, it was present in exports to Excel but not any more.

    Please help, I need this for printing urgently

    I see that the Date of Testing field does exist on the form. Since the account is enabled with HIPAA compliance, it is not possible for us to check the data. Could you check if the date field is being set in the PDF document from the PDF editor? If it isn't there, you may add the field from the PDF editor.
