Submissions: data is empty

On the last page of PDF, The only information we can find from the document is his signature, nothing else.

Looks like the form submission is empty, even if fields are set as required one. 

This can be a problem with the browser. If you know, can you please let us know what browser and operating system your user has used to fill the form? Would it be ok if we make a test submission directly on your form using the same browser to try to replicate the problem?
