    hi there,

Few questions as i'm comparing different hipaa compliant form builders

    1. Do you have documentation that you have been audited by a third party vendor (and have not failed in the past)

    2. Where is the data stored ?

    3. can i build graphs using data from these forms?

    thank you

    1. Do you have documentation that you have been audited by a third-party vendor (and have not failed in the past)

You may refer to our Security documentation to find more details about it: https://www.jotform.com/security/

After reading the page and you need more information or if there's a document you want to send, please contact the security support team by clicking the "Contact for more information" button. (You can find the image at the bottom of the security page)

    2. Where is the data stored ?

The HIPAA form data is stored in our HIPAA compliant servers which are separated from the Internet using multiple levels of protection and firewalls, and the encrypted data is stored in encrypted databases.

    3. can i build graphs using data from these forms?

Yes. Visual reports are available on HIPAA forms. Here's a guide on How-to-Create-a-Visual-Report-with-Your-Form-Submissions.

Unfortunately, the data will be pulled from multiple users. For example, in my screenshot below, the visual report is from 3 submissions/users.1597054833bar reports Screenshot 10
It is not possible to create a report for one user only.

can custom code be used to create a report for one individual user without
    violating hipaa?

Unfortunately, you can't use custom CSS to create a visual report. You can create a PDF submissions report if you want to display individual submission data in a report.

    Here's the guide: How-to-Customize-PDF-Submissions-Report

However, there is no report chart in the PDF document so you can't add a bar graph.

If you would like I can submit your requirement as a feature request to our developer team so that if there is a response on this, you will be notified here.

    Looking forward to your response.
