Unable to login to account.

Hello,

First off, my group does not have a paid subscription.

I believe one of our interns (who is no longer with us) created a form using jotform.  We have discovered a glitch, but I cannot log into the account.  It is the CECA Project Form with this link https://form.jotform.us/72144714888162

Is there any way that I can create a new login without losing this (our only) form?

Any help is appreciated.
