Change the look of the form

Hello I want to create form like this. https://form.jotform.com/kentzo/nextdaydelivery

I am unable to create header like in this form. In sample form header has two color shade and its width is small. I am sending my form link and form header screen shot.

To get any custom color gradient for your header please go to this site: https://cssgradient.io/ , which will give you the code you have to paste to the custom CSS field.

To add a clickable image, please follow this guide:

1. Upload your image(s) with using the "insert image". You can see "icon" of insert image on the below screenshot.
2. After select your image in text editor (select your image right to left like a text), click to link icon. After that "Insert Link" box will be pop. Now you can write your link for image.
3. Now when your image is clicked, users can go to link.
