Want donation options and ability to add custom amounts

Hi - i need to create a donation form with predefined amounts as well as the ability to add a custom amount instead of choosing one of the predefined amounts. 

On my form i can see this on the builder but when i publish its just the custom amount and i dont see the predefined boxes

I have checked your form and It doesn't show the predefined amount or boxes are setup on the back end.
