Hi,
thank you for reaching out to us.
1 - Regardless of the plan, all forms are protected with 256 bit SSL and you can choose to encrypt them for an added layer of protection. If you require HIPAA compliant forms, these are available on our Silver and Gold plans.
You can read more about security in general here and about HIPAA compliance here.
2 - JotForm servers are co-located in a cloud-based architecture with Google Cloud and Amazon Web Services (AWS). Google Cloud data centers are hosted in Iowa (US). AWS data centers are located both in Germany, Frankfurt (EU) and US, Virginia (US).
3 - Yes, you can use your forms to book appointments, for this simply add the Appointment Widget to your form.
4 - If you don't want to create the forms yourself, we generally recommend to post a job on a freelancing site like Upwork.
I hope this helps, please let us know if you have further questions.

st.george.pharmaceuticalReplied on September 18, 2020 at 12:28 PMAs per canadian requirements, the data residency need to be on canadian
servers.
There is something sync.com , which is a Canadian cloud service.
Is there a possibility that I can have, somehow, the data entered on the
forms to be taken into that Canadian cloud instead of staying on your
servers?

Hi,
since our standard plans run on shared servers this is nor achievable.
However, we do have the JotForm Enterprise plan where you get a dedicated server and in this case, you can have Canadian data residency.
If you would be interested in this, please contact our sales team by filling out the contact form here:

st.george.pharmaceuticalReplied on September 22, 2020 at 2:28 AMMy question is : is there anything in the design of the form that, once
filled out, it can have a copy sent to another destination, while the
original copy gets deleted , and that way we can satisfy the requirements
without having to get to the enterprise level , since I'm a solo
practitioner...

Hi,
thank you for providing more details.
You can easily set up an Autoresponder email that gets automatically sent to the customer or patient. You can even customize a PDF document with our PDF Editor and attach this to the email with the entered submission data.
We also have an Auto Delete Submissions Application that you can use to automatically delete the submissions on the JotForm end.
I hope this helps, I have moved your second question into a separate thread and will be addressing it here:

We don't have a direct integration with such could service but it is possible to pass the submission data to your MYSQL database.
Here is a guide:https://www.jotform.com/help/126-How-to-send-Submissions-to-Your-MySQL-Database-Using-PHP

In other words, will I be able to see separate applications for separate
patients?
