The Sender Name and Email is coming from JotForm instead of the Name and Custom Sender Email

    Hello,

    I've got conditions set for emails to be sent to specific email address. However; now I am getting my Notification email from Jotform and NOT the Supervisors Name or Email. Also, the Autoresponder is missing a field.

    I'd like my notifications to come to me once submitted by the Supervisors name and email based off of the condition I've got set for each one. 

    Second, I'd like to Autoresponder to be sent to that Supervisor also.

    Are you referring to the following form, https://www.jotform.com/202579196442058? If yes, you have no email conditions in the form - if that's what you mean by mentioning "conditions". You have two (2) notification emails in this form. One that is labeled as SUPERVISOR and another labeled as OFFICE ADMIN.

    The SUPERVISOR template is set to have its Sender Name to the Supervisor field of the form. The OFFICE ADMIN template's Sender Name is set to the Completed By field. Both have no Reply-To Email, and both are set to send from your custom sender email which is mreid@rainbow.ca.

    You would need to add the conditions if you want to conditionally sent both templates.

    As for the autoresponder, you do not have it in the form. Add one by following the steps from the below guide:

    I had to delete those conditions as each submission as being sent to every person I had a condition set to. 

    The Autoresponder, there is no email field to use. I'd like to it be sent to the person whom filled out the form

    When you set up an email condition, the default recipient of the form is no longer being used. The email is sent based on the logic, and it is sent to the list of recipients in that condition.

    I forgot to check the form fields a while ago. I see you do not have an email field in the form. The autoresponder template will require an email field as the recipient. You can't set up an autoresponder email without the email field.
