Type on an image

How can I create question type like below 2 sample in JotForm such that can type answer in the white boxes?

If you're familiar with CSS, you can use the position property to place four Short Text elements on top of the image for the circle one.

For the table, you can use Input table with custom CSS, specifically background-color, applied on the first row.
