It looks like the format of date is not follow as what i had set via jotform which is Y/M/D

    This one I'm using my own ID. New issue arise, it looks like the format of date is not follow as what i had set via jotform which is Y/M/D. and if you see the last doctor, the date even start with M/D/Y .

    Your form has an issue with the appointment slot widget, we can pass it on to our backend team for further checking, but I'm not sure when you will be updated because there are many support tickets in the queue.

    I think it would be better if you clone my demo form and see if it works for you. However, if the issue persists in the clone form version please let us know.

    I'm trying out using the clone form but i dont know why after some times, the date suddenly became not correct like the same issue arise again. why is it always happened like that?like sometimes the date is accurate, sometimes it become 1 day late.

    I just tried accessing your form and seems to work as intended. As you can see here, the date is in Y/M/D format the way you have set it up:

    Would it be a cache issue on your end? Could you please try accessing the form on an incognito window? See if it behaves the same.
