    I want to use this form on our dentistry website to collect sensitive information like SSN. Is this safe and secure? Is it all HIPAA compliant?

    Is there a phone number I can call?

    Yes, JotForm does offer HIPAA compliant accounts for our users that work in the medical field and are receiving and storing PHI(Protected Health Information).     Also, in regards to collecting SSN in your forms. Please note that it is against JotFrom Terms of Use to collect private information such as Social Security Numbers, Driver's License information, or other government-issued documents without following the proper security protocols.

    Here is a list of the steps needed to legally collect this information.

    1) Insert a Terms & Conditions widget or Short Scrollable Terms widget to your form with YOUR Terms of Use

    2) Insert any E-Signature widget/field into your form so your respondents can acknowledge that they are providing sensitive information
