How to show all the user's answer on the email notification?

hi,

i created a form with many medical questions with multipile choice for our patients.

i would like to get all the answers right to my mail but i'm gettion only partial and need to download the full submmision from the submmision area.

its a bit encumbered.

is ther a way to get all of the answers in the mail after the patient fill the form?

Hello Yael,

Yes, you can customize the email template on your form by using the EDIT TOOLBAR. Then click on the Form Fields option to add the fields that you would like to show on the email notification:

However, the EMAIL CONTENT - By default, all templates come in a table format, and the labels and input fields will be displayed inside this 2-column table. This is where most of the work will be done when customizing the template.
