We are doing some research to find a company to implement our online form completion on our website. We are a medical office and have a initial visit questionnaire that has diagram where patients can indicate on the body where their pain is at and use different symbols based on the type of pain.

Is this something that your forms can help us to achieve?

Using a widget like the Draw on Image Widget, you can have your users draw on your uploaded pain indicator diagram and use the provided drawing tools to replicate the symbols you have provided. Here's a quick example of how the widget works:

Yes. Sorry about the duplicate threads. I didn’t see that the first one went through.
I did see that widget, but I am wondering are we able to create different symbols for them to choose from or are we limited to what this one is? For example, I have attached our form that shows the different symbols we use. Is this something that can be done?

Please note that we do not have all of the exact symbols that you have, however, you may want to check out our Annotate Picture widget which has more options and see if you can work out with the tools that is within the widget.
