Why is the layout of our forms changed on some and not others?

Hello!

I've been back and forth about the layout settings with the new tables update that I did not want to use.

Now- some forms don't show the full legal name in the right place and it doesn't show on the list either- can someone confirm why this is happening and how to fix it so it's consistent?
