Display Unique ID in Dynamic QR widget

I'd like to use the Dynamic QR code to generate a QR code using the Accession Number that is automatically generated. I typed the name of the field "Accession Number" in the Dynamic QR code field but it didn't generate a QR code. Did I do something wrong ? I was able to generate the "Name" field QR code.

One other question too, can two fields be used to generate one QR code ? For example, Name and DOB in one QR code ?

Accession Number is a Unique ID widget, and this widget generates value after the form is submitted. This is done in such a way to prevent double unique numbers. Due to this, it can't be viewed on the form, used with a QR widget, or merged with other values.

You can combine two values using the Calculation widget, however, it can't be used with Unique ID.

If you wish, you can show the Accession Number on the thank you page, however, it's not possible to display the QR code there.
