forms that suits my needs

hi

i've been looking everywhere for a solution to my needs.

i need to send a form to all of my suppliers. i need them to fill out the form on a monthly basis place an order. i need to get all the responses in an excel format.

i need EACH response to be an excel file. i need each response (meaning excel file) to be delivered to a mail box so the backend will deal with the orders as seamlessly as possible.

i need to have the option to edit the form (for a new product etc.)
