Widget not working when uploaded to website

Hi

Something strange has just happened! The form has been working fine, both on Jotform and the website. But all of a sudden one of the sections has stopped working on the website. In fact you can't see the configurable list at all! Its as though the widget has stopped working!

This is a screenshot of the website and I have uploaded a screenshot of the form on Jotform below.

Please assist.

Could you please try embedding the form using the iFrame embed code? You can try following this guide: https://www.jotform.com/help/148-getting-the-form-iframe-code
