I am getting votes in, and when I check the mail I can see them, but there is no information about who they are. if this is done through facebook is that information gone?
I created another test case in which I sent the form via email and the email of both people show up in the list.

Hello @bubbabarlow,
Thank you for contacting JotForm Support.
I have checked the form you shared with us and it seems that you only have 1 question included in the form. If you want to get more information from your submitters, you may want to include a name and/or email address fields so you can match the vote to the person.

Hello @bubbabarlow,
Thank you for sharing your screenshot.
So, the reason you are not seeing the "Form Elements" is that you are actually in your Report Builder and that will not allow you to add fields to your form. If you want to edit the form, you will need to access the Form Builder instead.
Here is a screen recording to demonstrate how you can navigate between your Report Builder and Form Builder and other pages as well.

OK, I think we are not talking about the same thing. my form is fine the way it is. what I want is survey results that show the email address or even the name, or something, of the person who took the survey. I would have thought that this would go into the report. But I wonder, if it is linked through facebook, that this information might not be available.

Since there is no email field or full name field in the form, there is no way you can collect the submitter's info by setting the form as PUBLIC.
What I can suggest is to make it PRIVATE:
Then add the users as your form assignees:
Once they receive the invitation to submit, they will be required to create a Jotform account using their email address to view and submit the form. Or if they already have a Jotform account associated with their emails, they will be required to log in.
And as a result, every time they submit the form, you can identify who submitted it.
