Mystery Question appearing in email after submission

    I have a form that, when submitted, has a mystery question included after it is submitted.

    I have attached the an email of the form after submission with the additional question included. I have also provided the URL of the form as it appears when the user is filling it out - the question does not appear on the original form. Thank you.

    The mystery question appears 2/3rds of the way in between the "exposure" question and the "cage" question....thanks.

    Hi! The question "how frequently is your bird handled and does it ever go outside?" is in your email alert templates.

    It is possible that you created this question in your form first and then deleted it. Anyways, following is how you can find this in your email alert templates and delete it.

    After deleting this question from your email templates, please clear your form cache as well.
