Can you use look ups from database tables or store data to database tables which can be exported for reporting on results?

Unfortunately, no, we do not connect to any external database. You may however export your data into an excel file.

Then add Spreadsheet to Form widget to your form and import the excel file.

Users can then use that field to lookup for the rest of the data.
