    Your account was temporarily suspended since it was potentially hacked.

    There was a phishing form in your account.

    I have purged that form from your account and activated your account.

    I would recommend that you change the password on your account with immediate effect. You can change it from your Account Profile page or you can follow the reset instructions here: https://www.jotform.com/help/489-how-to-reset-jotform-account-password
