Attach form to linktree

I’m trying to make an assessment form for my credit repair business. Every time I make the full website and go through everything, it completely deletes after it says it saved. Why does that keep happening and what can I do? I’m trying to attach the form I create to my linktree so my clients can fill out this form to work with me.
