I tried logging in but it tells me I am unauthorized

    I try to login to my account the web browser tell me unauthorized and I login using my email they said no information

    Hello! You may be using wrong email address to login. I see two accounts which might belong to you; one is registered with li**k@ea**do*.com and another is registered with Ch**lo**e@ny**ma**he**th.com.

    If you're referring to one of these accounts; then I'll suggest you to reset your password and then try again.

    In case you're referring to a different account; then please provide us a Form ID/URL under that account; we'll try to find it.

    It's a Form User account, which means you clicked a Save button on a HIPAA form. You reset your password based on our logs. Please go ahead and click the Session Link. You'll be asked to sign in, do that to see if you still can't.

    Unfortunately, this type of account can't be searched on our end. You may consider creating an account with JotForm. Once you do that, go to My Forms page and click My Drafts section:
