Hello!
My form is showing hidden fields.
I also set this form up to send email notifications and it is not showing all of my fields as options for my email notifications.

I have checked the form you linked and I see that you have set up conditions in your form to show/hide field(s) based on the selected answer on the What Type of Order Would You Like to Place? field on your form.
Can you tell us what specific field(s) on your form is/are being shown when it should not when an option in that field is selected, please?
As for the content of the email notification in your form, I have noticed that not all fields in your form are included.

or, you can also just create a new which will have all the fields on your form by default.

As I have checked your form again, I found that the field elements you have mentioned are all supposed to be hidden when any of the following options are selected:
- Product Review Samples (to a buyer)
- Signage (to a store)
- Janitorial Supplies (to a store)
In addition to that, the Store Event section on your form is also supposed to be hidden when any of the following options are selected:
- Testers (to a store)
- Free Fill (to a store)
Upon checking the Condition Settings of your form, I see that you have created a condition with different "if rules" for each option.
To fix the issue you are having, you would have to include all the "if rules" that does the same thing when met in one condition. Here's an example.
