Can we create a check off box for office use?

    Is there a way to create a check off box for office use only?

    Hi,

    As for your question regarding adding "Office Use Only" with the check boxes, You can set up check off boxes first in your form;

    And you should add "Office Use Only" by adding paragraph field and inserting the text there at the bottom. (or hide the part if necessary per https://www.jotform.com/help/434-how-to-hide-form-fields)
