Transfer Form to Another Account Did Not Work

I previously requested the following and the user does not see where the form transferred to their account. Can you please assist?

"Can you please transfer the GTA Bi-Weekly Report and the GTA Evaluation forms from my account (username: kgarrity) to another account (username: kaylagwilliams)?"
