Can I create a dynamic form that will bring name and prior donations from a spreadsheet

I want to create a form that will brings in the donors name (from an excel spreadsheet is fine) along with previous giving history. Like their 2019 pledge amount. I also want to be able to suggest current giving amounts based on a 10% increase, 20% etc. That can be in the spread sheet also.

Is it called a dynamic for or some other name? Is this possible
