I checked from our backend, currently you are on GUEST account and no form. No JotForm account associated with kparker@pelitas.com
Perhaps you are referring to another JotForm account that you have.
Could you kindly share us the form URL?

Regarding "Submit button not showing on live form"
While we wait for the form URL, could you please check your form conditional logic? Kindly check for an IF condition that prevents the Submit button to show on the form.
