What has happened to my forms?!

I am with Cocker Spaniel Rescue of East Texas. We have been using JotForm for over a year. Our form still works (https://www.cockerkids.org/copy-of-adoption-process-2 ) The username is CSRForms. As recently as Feb 1, I could log into JotForms and see the submitted forms. Now everything is gone and the page says we have no forms. What has happened?
