Form Changed Itself

Hello, somehow one of my Senior Boxes form edited itself where everything is almost erased.

I did it but under the revision history it shows only one change and that's the current form. This is odd as I made a number of revisions before.

Did you perhaps view the wrong form?

This one (https://www.jotform.com/form/210020815021130) uses the same PDF document and has more fields in it.
