Form shows error even field is already filled.

Hello,

After trying for about 3 hours even when the fields are filled the form shows the error attached in the image, is a form that is not created by me, then, I cannot modify it.

Please, allow me to know what should I do to solve the situation is very important information which I need to process asap.
