Missing field when preview

I have a form that I built that has missing/added fields when I publish it. Below is how I want it to be. The additional attached picture shows that the "client Code" field is missing. I am unable to attach a third picture, but a "fill in the blanks" field is added in the published form. Please help me to figure out what is going on here!
